Travel policies bought in Spain exclude Ukraine. Almost every Spanish insurer excludes countries the Foreign Ministry advises against visiting, and Ukraine has been on that list since February 2022. Even if your policy does not exclude it explicitly, the Ukrainian border will not accept it.
Article 7 of Ukraine's Insurance Law requires every foreigner to present, on entry, a policy issued by an insurer licensed in Ukraine. The State Border Guard checks the document, the dates and the cover before stamping your passport.

Yes. The Ukrainian State Border Guard requires all foreigners, EU citizens included, to hold a medical insurance policy from a Ukrainian-licensed insurer. Without it you can be refused entry.
No. Spanish citizens enter Ukraine visa-free for up to 90 days in any 180-day period. You only need a valid passport and the insurance.

Yes, if your previous policy has expired or you entered without one. Choose today as the start date. Holding valid insurance for the whole stay is also mandatory, not only at the border.
Before the start date you can cancel for a refund and buy a new policy with the correct dates. Once cover has started, the policy cannot be changed.
Call the 24-hour assistance number printed on the policy. The assistance centre tells you which hospital to go to and arranges direct payment with it. Keep all receipts.
